This is a match worn Gonzalo Jara Reyes Nottingham Forest away shirt from the 2013/14 season. The shirt was obtained via his sponsors for that season in which they had got it personally from him at a sponsors dinner - there is a photo I have added from that evening. The shirt is a size M and is a rare long sleeve version which was not available publicly.
